The Eight Data Protection Act Principles followed by Hull Hypnotherapy

The act contains eight "Data Protection Principles". These specify that personal data must be:

1. Processed fairly and lawfully - Hull Hypnotherapy will collect information such as your name, date of birth, address, telephone number, GP contact details and information about the issue you are visiting us with inorder for us to get a bigger picture of what we are dealing with.

2. Obtained for specified and lawful purposes - The information gathered by Hull Hypnotherapy will be used to assess the best possible therapies for your issue or condition, and also to estimate the amount of sessions required.

3. Adequate, relevant and not excessive - Hull Hypnotherapy will make notes on the relevant issues relating to the condition you present with, it will not be an excessive consultation but it will be thorough depending on your level of disclosure.  Sensitive information is not recorded without your consent.

4. Accurate and up to date. - Consultations will be reviewed each session, this will be your treatment plan. You view the treatment plan and consultation during each session and sign to agree its contents.

5. Not kept any longer than necessary. - Consultation forms are kept for the number of years required by the insurers and regulating bodies although not disclosed to them.

6. Processed in accordance with the "data subject's" (the individual's) rights. - Information collected by Hull Hypnotherapy will not be disclosed to a third party without your consent. Or if the therapist believes you to be a danger to yourself or another person.

7. Securely kept. - In a file in the centre to be accessed by your therapist only

8. Not transferred to any other country without adequate protection in situ. - As above.
